We start with a comprehensive exam to rule out tooth decay, gum disease, an old failing filling, or a sinus or tonsil source. A tongue scrape and a periodontal probing chart usually reveal where the smell is coming from.

Treatment is targeted: a thorough cleaning to clear plaque and tartar, a tongue-hygiene plan, restorative work on any failing filling or crown, and periodontal therapy if pockets are involved. A short course of medicated rinse may be added for stubborn cases.
